COH-FIT is an online, international, anonymous survey measuring changes in well-being (WHO-5) and a composite psychopathology P-score, and their associations with COVID-19 deaths/restrictions, 12 a-priori defined risk individual/cumulative factors, and coping strategies during COVID-19 pandemic (26/04/2020-26/ 06/2022) in 30 languages (representative, weighted non-representative, adults). T-test, χ2, penalized cubic splines, linear regression, correlation analyses were conducted. Analyzing 121,066/142,364 initiated surveys, WHO-5/P-score worsened intra-pandemic by 11.1±21.1/13.2±17.9 points (effect size d=0.50/0.60) (comparable results in representative/weighted non-probability samples). Persons with WHO-5 scores indicative of depression screening (<50, 13% to 32%) and major depression (<29, 3% to 12%) significantly increased. WHO-5 worsened from those with mental disorders, female sex, COVID-19-related loss, low-income country location, physical disorders, healthcare worker occupations, large city location, COVID-19 infection, unemployment, firstgeneration immigration, to age=18-29 with a cumulative effect. Similar findings emerged for P-score. Changes were significantly but minimally related to COVID-19 deaths, returning to near-pre-pandemic values after >2 years. The most subjectively effective coping strategies were exercise and walking, internet use, social contacts. Identified risk groups, coping strategies and outcome trajectories can inform global public health strategies.01A - Beitrag in wissenschaftlicher ZeitschriftPublikation Förderung von AI Literacy als Schlüsselkompetenz. Das Projekt bietet somit einen innovativen Ansatz, um Studierende und Dozierende auf die Anforderungen der KI-Ära vorzubereiten, und fördert die Entwicklung von AI Literacy als Schlüsselkompetenz.01A - Beitrag in wissenschaftlicher ZeitschriftPublikation Assessing the quality and impact of eHealth tools: systematic literature review and narrative synthesis(JMIR Publications, 2023) Jacob, Christine; Lindeque, Johan Paul; Klein, Alexander; Ivory, Chris; Heuss, Sabina; Peter, Marc K.Background: Technological advancements have opened the path for many technology providers to easily develop and introduce eHealth tools to the public. The use of these tools is increasingly recognized as a critical quality driver in health care; however, choosing a quality tool from the myriad of tools available for a specific health need does not come without challenges. Objective: This review aimed to systematically investigate the literature to understand the different approaches and criteria used to assess the quality and impact of eHealth tools by considering sociotechnical factors (from technical, social, and organizational perspectives). Methods: A structured search was completed following the participants, intervention, comparators, and outcomes framework. We searched the PubMed, Cochrane, Web of Science, Scopus, and ProQuest databases for studies published between January 2012 and January 2022 in English, which yielded 675 results, of which 40 (5.9%) studies met the inclusion criteria. The PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines and the Cochrane Handbook for Systematic Reviews of Interventions were followed to ensure a systematic process. Extracted data were analyzed using NVivo (QSR International), with a thematic analysis and narrative synthesis of emergent themes. Results: Similar measures from the different papers, frameworks, and initiatives were aggregated into 36 unique criteria grouped into 13 clusters. Using the sociotechnical approach, we classified the relevant criteria into technical, social, and organizational assessment criteria. Technical assessment criteria were grouped into 5 clusters: technical aspects, functionality, content, data management, and design. Social assessment criteria were grouped into 4 clusters: human centricity, health outcomes, visible popularity metrics, and social aspects. Organizational assessment criteria were grouped into 4 clusters: sustainability and scalability, health care organization, health care context, and developer. Conclusions: This review builds on the growing body of research that investigates the criteria used to assess the quality and impact of eHealth tools and highlights the complexity and challenges facing these initiatives. It demonstrates that there is no single framework that is used uniformly to assess the quality and impact of eHealth tools. It also highlights the need for a more comprehensive approach that balances the social, organizational, and technical assessment criteria in a way that reflects the complexity and interdependence of the health care ecosystem and is aligned with the factors affecting users’ adoption to ensure uptake and adherence in the long term.01A - Beitrag in wissenschaftlicher ZeitschriftPublikation Der Ton macht die Musik(EMH Schweizerischer Ärzteverlag, 2023) Barbosa, Sonia; Heuss, SabinaIn Spitälern treffen heute drei Generationen mit unterschiedlichsten Funktionen aufeinander. They hold a team together, help hospitals to develop and retain capable employees, and are directly related to physicians’ job satisfaction. We aim to test the impact of leadership models and various items of leadership communication on job satisfaction and well-being of physicians working in Swiss hospitals. Method We conducted a nationwide survey of physicians in Swiss hospitals in 2019 to assess the correlation between communication and leadership style with job satisfaction and well-being of physicians. The evaluation of the survey uses multivariate, statistical analysis (descriptive evaluations, regression model, factor analyses). Results The data reveal that leadership communication, e.g., the tone of communication, information quality and information behaviour and feedback have the most positive impact on job satisfaction and well-being. These items have a stronger influence on well-being when physicians belong to a younger generation, such as Generation Y (born after 1980). Leadership communication has the greatest impact on job satisfaction on residents and a lesser impact on physicians in higher hierarchical levels. Conclusions To increase job satisfaction and well-being amongst physicians, hospitals superiors and supervisors are advised to invest in education and training on leadership communication and feedback with special focus on the younger generation as well as on female physicians as this will have the greatest impact.01A - Beitrag in wissenschaftlicher ZeitschriftPublikation Patients' preference regarding inpatient versus outpatient setting ‐ A systematic review(Wiley, 2023) Schneeberger, Andres R.; Werthmüller, Sarah; Barco, Santiago; Heuss, SabinaOBJECTIVES Continuously rising healthcare costs have led to financial pressure on the healthcare systems. One of the trends for the reduction of costs is the shift towards outpatient treatment. However, research has not focused on the patients' preferences regarding inpatient versus outpatient treatment settings. The purpose of this review is to examine existing studies surveying patients' preferences related to inpatient and outpatient treatment methods. The aim is to find out whether patients' wishes were queried and considered in the decision-making process. DESIGN Therefore, the reviewers performed a systematic approach utilizing the PRISMA standards and screened 1'646 articles out of 5'606 articles from the systematic search. RESULTS AND CONCLUSION The screening resulted in 4 studies that analyzed exclusively the patient's choice of treatment setting. The search showed an apparent paucity of current literature and highlights the need for further research. The authors' recommendation includes a better involvement of patients in the decision-making process as well as adding preferred treatment settings to advanced treatment directives and patient satisfaction questionnaires.01A - Beitrag in wissenschaftlicher ZeitschriftPublikation A sociotechnical framework to assess patient-facing eHealth tools. results of a modified Delphi process(Nature, 2023) Jacob, Christine; Lindeque, Johan Paul; Müller, Roman; Klein, Alexander; Metcalfe, Thomas; Connolly, Samantha L.; Koerber, Florian; Maguire, Roma; Denis, Fabrice; Heuss, Sabina; Peter, Marc K.Among the thousands of eHealth tools available, the vast majority do not get past pilot phases because they cannot prove value, and only a few have been systematically assessed. Although multiple eHealth assessment frameworks have been developed, these efforts face multiple challenges. This study aimed to address some of these challenges by validating and refining an initial list of 55 assessment criteria based on previous frameworks through a two-round modified Delphi process with in-between rounds of interviews. The expert panel (n = 57) included participants from 18 countries and 9 concerned parties. A consensus was reached on 46 criteria that were classified into foundational and contextual criteria. The 36 foundational criteria focus on evaluating the eHealth tool itself and were grouped into nine clusters: technical aspects, clinical utility and safety, usability and human centricity, functionality, content, data management, endorsement, maintenance, and developer. The 10 contextual criteria focus on evaluating the factors that vary depending on the context the tool is being evaluated for and were grouped into seven clusters: data-protection compliance, safety regulatory compliance, interoperability and data integration, cultural requirements, affordability, cost-benefit, and implementability. The classification of criteria into foundational and contextual helps us assess not only the quality of an isolated tool, but also its potential fit in a specific setting. Criteria subscales may be particularly relevant when determining the strengths and weaknesses of the tool being evaluated. This granularity enables different concerned parties to make informed decisions about which tools to consider according to their specific needs and priorities.01A - Beitrag in wissenschaftlicher ZeitschriftPublikation ‘Social’ media? The aim of this study was to test concurrently the validity of COH-FIT items and the internal validity of the co-primary outcome, a composite psychopathology “P-score”. The COH-FIT survey has been translated into 30 languages (two blind forward-translations, consensus, one independent English back-translation, final harmonization). To measure mental health, 1–4 items (“COH-FIT items”) were extracted from validated questionnaires (e.g. Patient Health Questionnaire 9). COH-FIT items measured anxiety, depressive, post-traumatic, obsessive-compulsive, bipolar and psychotic symptoms, as well as stress, sleep and concentration. COH-FIT Items which correlated r ≥ 0.5 with validated companion questionnaires, were initially retained. A P-score factor structure was then identified from these items using exploratory factor analysis (EFA) and confirmatory factor analyses (CFA) on data split into training and validation sets. Consistency of results across languages, gender and age was assessed. From >150,000 adult responses by May 6th, 2022, a subset of 22,456 completed both COH-FIT items and validated questionnaires. Concurrent validity was consistently demonstrated across different languages for COH-FIT items. CFA confirmed EFA results of five first-order factors (anxiety, depression, post-traumatic, psychotic, psychophysiologic symptoms) and revealed a single second-order factor P-score, with high internal reliability (ω = 0.95). Factor structure was consistent across age and sex. COH-FIT is a valid instrument to globally measure mental health during infection times. The P-score is a valid measure of multidimensional mental health.01A - Beitrag in wissenschaftlicher Zeitschrift
