The objectives of this study are to describe the variations in annoyance due to air pollution in Europe and its individual and environmental determinants. Methods This study took place in the context of the European Community Respiratory Health Survey II (ECRHS II) that was conducted during 1999–2001. It included 25 centres in 12 countries and 7867 randomly selected adults from the general population. Annoyance due to air pollution was self-reported on an 11-point scale. Annual mean mass concentration of fine particles (PM2.5) and its sulphur (S) content were measured in 21 centres as a surrogate of urban air pollution. Results Forty-three per cent of participants reported moderate annoyance (1–5 on the scale) and 14% high annoyance (≥6) with large differences across centres (2–40% of high annoyance). Participants in the Northern European countries reported less annoyance. Female gender, nocturnal dyspnoea, phlegm and rhinitis, self-reported car and heavy vehicle traffic in front of the home, high education, non-smoking and exposure to environmental tobacco smoke were associated with higher annoyance levels. At the centre level, adjusted means of annoyance scores were moderately associated with sulphur urban levels (slope 1.43 μg m−3, standard error 0.40, r = 0.61). Conclusions Annoyance due to air pollution is frequent in Europe. Individuals’ annoyance may be a useful measure of perceived ambient quality and could be considered a complementary tool for health surveillance.01A - Beitrag in wissenschaftlicher ZeitschriftPublikation Associations between social integration, participation and productivity loss among persons with chronic pain: a registry based cross sectional study(BioMed Central, 2022) Friedli, Tom; Brodbeck, Jeannette; Gantschnig, Brigitte E.Purpose To examine associations between factors of social inclusion and participation and productivity loss in employed persons with chronic pain, assessed for an interprofessional pain rehabilitation programme. We hypothesized that factors of social inclusion and participation and work related social factors are significantly associated with productivity when experiencing chronic pain and we expected a moderate effect. Methods Cross-sectional study using data collected prospectively in an interprofessional patient registry for chronic pain. The primary end point was productivity loss, measured with the iMTA Productivity Costs Questionnaire. We included data from 161 individuals. To be included, persons had to be 18 years old or older, in paid work, and had to have a medical diagnosis of chronic pain syndrome with actual or potential tissue damage. In addition, participants had to have indicators of significant impairments in psychosocial functions. Results Linear regression analysis showed that a highly stressful professional situation, frequent problems regarding the compatibility of the family and job and not being Swiss were associated with a significantly higher total productivity loss. Similar results were found for productivity loss in paid work. However, problems concerning the compatibility of the family and job did not reach the significance level for productivity loss in paid work. Conclusion The results of this study underscore the importance of factors of social inclusion and participation for interprofessional rehabilitation programmes to manage chronic pain especially when focussing on productivity loss.01A - Beitrag in wissenschaftlicher ZeitschriftPublikation Characterization of source-specific air pollution exposure for a large population-based Swiss cohort (SAPALDIA)(National Institute of Environmental Health Sciences, 2007) Liu, L.-J. Methods: We evaluated total and traffic-specific particulate matter < 10 and < 2.5 microm in aero-dynamic diameter (PM(10), PM(2.5)), nitrogren dioxide, and nitrogen oxide concentrations predicted by Gaussian dispersion models against fixed-site measurements at different locations, including traffic-impacted, urban-background, and alpine settings between and across cities. The model predictions were then used to estimate individual subjects' historical and cumulative exposures with a temporal trend model. Results: Modeled PM(10) and NO(2) predicted at least 55% and 72% of the variability of the measured PM(10) and NO(2), respectively. Traffic-specific pollution estimates correlated with the NO(x) measurements (R(2) >or=0.77) for background sites but not for traffic sites. Regional background PM(10) accounted for most PM(10) mass in all cities. Whereas traffic PM(10) accounted for < 20% of the total PM(10), it varied significantly within cities. The modeling error for PM(10) was similar within and between cities. Traffic NO(x) accounted for the majority of NO(x) mass in urban areas, whereas background NO(x) accounted for the majority of NO(x) in rural areas. The within-city NO(2) modeling error was larger than that between cities. Conclusions: The dispersion model predicted well the total PM(10), NO(x), and NO(2) and traffic-specific pollution at background sites. However, the model underpredicted traffic NO(x) and NO(2) at traffic sites and needs refinement to reflect local conditions. Diesel exhaust particles (DEP) contain elemental carbon (EC), the dominant light-absorbing substance in the atmosphere. Black smoke (BS) is a measure for light absorption of PM and, thus, an alternative way to estimating EC concentrations, which may serve as a proxy for diesel exhaust emissions. We analyzed PM2.5 and BS data collected within the EXPOLIS study (Air Pollution Exposure Distribution within Adult Urban Populations in Europe) in Athens, Basel, Helsinki, and Prague. 186 indoor/outdoor filter pairs were sampled and analyzed. PM2.5 and BS levels were lowest in Helsinki, moderate in Basel, and remarkably higher in Athens and Prague. In each city, Spearman correlation coefficients of indoor versus outdoor were higher for BS (range rSpearman: 0.57−0.86) than for PM2.5 (0.05−0.69). In a BS linear regression model (all data), outdoor levels explained clearly more of indoor variation (86%) than in the corresponding PM2.5 model (59%). In conclusion, ambient BS seizes a health-relevant fraction of fine particles to which people are exposed indoors and outdoors and exposure to which can be assessed by monitoring outdoor concentrations. The psychological effects of air pollution may often be more important to well-being than the biophysical effects. Health effects of perceived annoyance from air pollution are so far unknown. More knowledge of air pollution annoyance levels, determinants and also associations with different air pollution components is needed. In the European air pollution exposure study, EXPOLIS, the air pollution annoyance as perceived at home, workplace and in traffic were surveyed among other study objectives. Overall 1736 randomly drawn 25–55-yr-old subjects participated in six cities (Athens, Basel, Milan, Oxford, Prague and Helsinki). Levels and predictors of individual perceived annoyances from air pollution were assessed. Instead of the usual air pollution concentrations at fixed monitoring sites, this paper compares the measured microenvironment concentrations and personal exposures of PM2.5 and NO2 to the perceived annoyance levels. A considerable proportion of the adults surveyed was annoyed by air pollution. Female gender, self-reported respiratory symptoms, downtown living and self-reported sensitivity to air pollution were directly associated with high air pollution annoyance score while in traffic, but smoking status, age or education level were not significantly associated. Population level annoyance averages correlated with the city average exposure levels of PM2.5 and NO2. A high correlation was observed between the personal 48-h PM2.5 exposure and perceived annoyance at home as well as between the mean annoyance at work and both the average work indoor PM2.5 and the personal work time PM2.5 exposure. With the other significant determinants (gender, city code, home location) and home outdoor levels the model explained 14% (PM2.5) and 19% (NO2) of the variation in perceived air pollution annoyance in traffic. Compared to Helsinki, in Basel and Prague the adult participants were more annoyed by air pollution while in traffic even after taking the current home outdoor PM2.5 and NO2 levels into account.01A - Beitrag in wissenschaftlicher ZeitschriftPublikation Evaluation of VOC measurements in the EXPOLIS study(Royal Society of Chemistry, 2001) Jurvelin, Jouni; Edwards, Rufus; Saarela, Kristina; Laine-Ylijoki, Jutta; De Bortoli, Maurizio; Bayer-Oglesby, Lucy; Schläpfer, Kurt; Georgoulis, Lambros; Tischerova, Eva; Hänninen, Otto; Jantunen, MattiPersonal exposures and microenvironment concentrations of 30 target VOCs were measured for 401 participants living in five European cities as a part of the EXPOLIS (Air Pollution Exposure Distributions within Adult Urban Populations in Europe) study. Measurements in Basel used an active charcoal (Carbotech) adsorbent as opposed to the Tenax TA used in the other study centres. In addition, within each centre, personal and microenvironment VOC sampling required different sampling pumps and, because of different sampling durations, different sampling flow rates. Thus, careful testing of the sampling and analysis procedures was required to ensure accuracy and comparability of collected data. Monitor comparison tests using Tenax TA showed a mean VOC concentration ratio of 0.95 between the personal and microenvironment monitors. The LODs for the target VOCs using Tenax TA ranged from 0.7 to 5.2 µg m−3. The LODs for the 14 target compounds quantifiable using Carbotech ranged from 0.9 to 3.2 µg m−3. Tenax TA field blanks showed no remarkable contamination with the target VOCs, except benzaldehyde, a known artefact with this adsorbent. Thus, the diffusion barrier system used prevented contamination of Tenax TA samples by passive diffusion during non-sampling periods. Duplicate and parallel evaluations of the Tenax TA and Carbotech showed an average difference of <17% in VOC concentrations within the sampling methods, but a systematic difference between the methods (Tenax TA ∶ Carbotech concentration ratio = 1.18–2.36). These field evaluations and quality assurance tests showed that interpretation and comparison of the results in any VOC monitoring exercise should be done on a compound by compound basis. It is also apparent that carefully planned and realised QA and QC (QA/QC) procedures are needed in multi-centre studies, where a common sampling method and laboratory analysis technique are not used, to strengthen and simplify the interpretation of observed VOC levels between participating centres.01A - Beitrag in wissenschaftlicher ZeitschriftPublikation Exploring online health information seeking and sharing among older adults: a mini-review about acceptance, potentials, and barriers(Frontiers Research Foundation, 2024) Bachofner, Yves; Seifert, Alexander; Sepahniya, Samin; Fabian, CarloOnline health information seeking (OHIS) is understood by health care, health promotion, and disease prevention experts as a resource for healthy aging. Nevertheless, empirical findings regarding the acceptance, potentials, and barriers of OHIS among older adults are limited. This mini-review aims to explore the level of acceptance of OHIS, including passive reading of information and active interactions with peers, among the older population. Furthermore, it examines the potentials and barriers associated with such practices. This paper presents the associations between the personal PM2.5 exposures, microenvironment (residential indoor, residential outdoor and workplace indoor) and ambient fixed site concentrations measured in Helsinki (Finland), Basel (Switzerland), Prague (Czech Republic) and Athens (Greece). Considering the whole chain from ambient fixed site to residential outdoor, residential indoor and personal leisure time (non-working hours) exposure, the correlations were highest between personal leisure time exposures and residential indoor concentrations (non-environmental tobacco smoke (ETS): Pearson r=0.72−0.92, ETS included: r=0.82−0.86) except in Athens, where the correlation between residential indoor and outdoor air was highest (non-ETS: r=0.82, ETS included: r=0.68)). Unfortunately, ambient fixed site PM2.5 concentrations were measured continuously only in Helsinki. Ambient fixed site PM2.5 concentrations correlated quite well with residential outdoor concentrations (r=0.90), and also with residential indoor (non-ETS) concentrations (r=0.80), but concentrations measured at ambient fixed site monitors were poor predictors of personal exposures to PM2.5. They were particularly poor predictors of personal workday exposures (non-ETS: r=0.34, ETS included: r=0.25), but considerably better for personal leisure time exposures (non-ETS: r=0.69, ETS included: r=0.54). Methods: The health assessment at recruitment (1991) and at the first reassessment (2001-3) consisted of an interview about respiratory health, occupational and other exposures, spirometry, a methacholine bronchial challenge test, end-expiratory carbon monoxide (CO) measurement and measurement for atopy. A bio bank for DNA and blood markers was established. Heart rate variability was measured using a 24-hour ECG (Holter) in a random sample of participants aged 50 years and older. Concentrations of nitrogen dioxide (NO2), sulphur dioxide (SO2), ozone (O3) and particulates in ambient air have been monitored in all study areas since 1991. Residential histories collected over the 11 year follow-up period coupled with GIS modelling will provide individual long-term air pollutant exposure estimates. Results: Of 9651 participants examined in 1991, 8715 could be traced for the cohort study and 283 died. Basic information about health status was obtained for 8047 individuals (86% of alive persons), 6 528 individuals (70%) agreed to the health examination and 5 973 subjects (62%) completed the entire protocol. Non-participants in the reassessment were on average younger than participants and more likely to have been smokers and to have reported respiratory symptoms in the first assessment. Objectives: We investigated whether decreases in particulates with an aerodynamic diameter of less than 10 microm (PM10) were associated with lower rates of reporting respiratory symptoms (i.e., decreased morbidity) on follow-up. Methods: The present analysis includes 7,019 subjects who underwent detailed baseline examinations in 1991 and a follow-up interview in 2002. Each subject was assigned model-based estimates of average PM10 during the 12 months preceding each health assessment and the difference was used as the exposure variable of interest (DeltaPM10). Analyses were stratified by symptom status at baseline and associations between DeltaPM10 and change in symptom status during follow-up were adjusted for important baseline characteristics, smoking status at follow-up, and season. We then estimated adjusted odds ratios for symptoms at follow-up and numbers of symptomatic cases prevented due to the observed reductions in PM10. Measurements and main results: Residential exposure to PM10 was lower in 2002 than in 1991 (mean decline 6.2 microg/m3; SD = 3.9 microg/m3). Estimated benefits (per 10,000 persons) attributable to the observed changes in PM10-levels were: 259 (95% confidence interval [CI]: 102-416) fewer subjects with regular cough, 179 (95% CI, 30-328) fewer subjects with chronic cough or phlegm and 137 (95% CI, 9-266) fewer subjects with wheezing and breathlessness. Therefore, time-microenvironment-activity data are particularly useful in modeling exposure. We investigated determinants of workday time-microenvironment-activity patterns of the adult urban population in seven European cities. The EXPOLIS study assessed workday time-microenvironment-activity patterns among a total of 1427 subjects (age 19-60 years) in Helsinki (Finland), Athens (Greece), Basel (Switzerland), Grenoble (France), Milan (Italy), Prague (Czech Republic), and Oxford (UK). Subjects completed time-microenvironment-activity diaries during two working days. We present time spent indoors--at home, at work, and elsewhere, and time exposed to tobacco smoke indoors for all cities. The contribution of sociodemographic factors has been assessed using regression models. More than 90% of the variance in indoor time-microenvironment-activity patterns originated from differences between and within subjects rather than between cities. The most common factors that were associated with indoor time-microenvironment-activity patterns, with similar contributions in all cities, were the specific work status, employment status, whether the participants were living alone, and whether the participants had children at home. Gender and season were associated with indoor time-microenvironment-activity patterns as well but the effects were rather heterogeneous across the seven cities. Exposure to second-hand tobacco smoke differed substantially across these cities. The heterogeneity of these factors across cities may reflect city-specific characteristics but selection biases in the sampled local populations may also explain part of the findings. We used home outdoor NO2 measurements taken from eight geographically diverse areas to examine spatio-temporal variations, construct, and evaluate models that could best predict the within-city contrasts in observations. Passive NO2 measurements were taken outside of up to 100 residences per area over three seasons in 1993 and 2003 as part of the Swiss cohort study on air pollution and lung and heart disease in adults (SAPALDIA). The spatio-temporal variation of NO2 differed by area and year. Regression models constructed using the annual NO2 means from central monitoring stations and geographic parameters predicted home outdoor NO2 levels better than a dispersion model. However, both the regression and dispersion models underestimated the within-city contrasts of NO2 levels. Our results indicated that the best models should be constructed for individual areas and years, and would use the dispersion estimates as the urban background, geographic information system (GIS) parameters to enhance local characteristics, and temporal and meteorological variables to capture changing local dynamics. The SAPALDIA study is a long-term air pollution study that included 7990 subjects from 8 areas in Switzerland with the first health examination in 1991 and the second examination in 2002. Each area was monitored with up to 3 monitoring sites for PM, NO2, and other gaseous pollutants. In 1999–2000, a sampling campaign was conducted for PM10, PM2.5, and black smoke at 16 Swiss sites. In 2002–2003, passive NO2 measurements were collected strategically over the year outside and inside approximately 60 homes per area. Annual average concentrations of source-specific and total PM2.5, PM10, and NOx were estimated using a Gaussian dispersion model with GIS to match individual residences of the SAPALDIA subjects. This paper examines the performance of the dispersion model, variation of source-specific air pollution exposures, and the implications of these findings to long-term air pollution epidemiologic studies. For PM10 in 2000, modeled values predicted 68% of the variability in the measurements. For NO2 in 2000, the model predicted the measured values with an R2 over 0.80. The R2 for traffic-specific pollutant predictions ranged between 0.44 (P = 0.08) for traffic-related PM2.5 and 0.81 (P < 0.01) for traffic-related NO2 for sites with low traffic impacts. However, when traffic sites were included in the comparisons, the R2 was lower, ranging between 0.41 for traffic-originated PM10 and 0.51 for traffic-originated NO2. Nevertheless, our preliminary results indicated that variance in traffic-originated pollutants accounted for up to 45% of the variance in total PM10, 69% of that in total PM2.5, and 91% of that in NOx. In addition, we smoothed actual NO2 measurements outside individual residences and correlated the resulting smoothed estimates at these sites with NO2 estimates from the dispersion model. To obtain good agreement between the measured and modeled surfaces (r > 0.60), the minimal spatial smoothing window was found to range between 200 m in rural Davos and 1.75 km for urban Basel. Our results indicate that sites affected largely by regional and urban background pollution are properly presented by the model. Locations impacted by local traffic, however, may not be adequately predicted by the model and need either fine-tuning of the model or additional parameters to reflect local conditions. Dieser Flyer ist die Grundlage, um in und mit Tessiner Gemeinden betreffend partizipativen und gesundheitsfördernden Projekten ins Gespräch zu kommen. Mit dem Flyer sollen Gemeinden informiert und sensibilisiert sowie schliesslich motiviert werden, entsprechende Projekte umzusetzen. Das Projekt wurde im Auftrag von und in Kooperation mit dem Ufficio del medico cantonale, Ticino (Kantonsärztlicher Dienst) umgesetzt. Der ursprüngliche Auftrag lautete, aus der bereits bestehenden Publikation «Partecipazione e promozione della salute» von Gesundheitsförderung Schweiz einen Flyer als Kurzform zu produzieren. In den Flyer sind schliesslich Erkenntnisse und praxisbezogene Informationen aus realisierten Projekten und wissenschaftlicher Literatur eingeflossen. Das neu entwickelte Handlungsmodell (Projektzyklus mit Hinweisen, wie jeweils partizipativ gearbeitet werden kann), bildet nun den Kern des neu entwickelten Flyers. The average residential outdoor and indoor NO2 levels were lowest in Helsinki (24 12 and18 11 mgm 3, respectively), highest in Prague (61 20 and43 23 mgm 3), with Basel in between (36 13 and27 13 mgm 3). Average workplace NO2 levels, however, were highest in Basel (36 24 mgm 3), lowest in Helsinki (27 15 mgm 3), with Prague in between (30 18 mgm 3). A time-weightedmicroenvironmental exposure model explained74% of the personal NO2 exposure variation in all centres and in average 88% of the exposures. Log-linear regression models, using residential outdoor measurements (fixedsite monitoring) combinedwith residential andwork characteristics (i.e. work location, using gas appliances and keeping windows open), explained48% (37%) of the personal NO2 exposure variation. Regression models based on ambient fixed site concentrations alone explained only 11–19% of personal NO2 exposure variation. Evaluation of participants with personal exposures above median levels in the EXPOLIS study in Athens, Helsinki, Oxford and Prague illustrated that these participants frequently showed a different relationship to indoor and workplace levels than that shown by the population median. Thus, prioritization of environments for control measures based on median exposures may exclude important areas where effectively focused control measures are possible, and may therefore have little impact on the highest and most harmful exposures. Further, personal exposures at the upper end of the distribution may exceed the US EPA inhalation reference concentration (Rfc), illustrated here using hexane, naphthalene and benzene. For example upper 90th percentile personal exposures to benzene in Athens and Prague were 64 and 27 μg m−3 with peak exposures of 217 and 38 μg m−3, respectively for non-ETS exposed participants relative to an Rfc of 30 μg m−3. Strategies to reduce exposures to individual compounds, therefore, may benefit from focus on the high end of the distribution to identify activities and behaviors that result in elevated exposures. Control strategies targeting activities that lead to exposures in the upper end of the distribution would reduce the variability associated with population median values by bringing the upper end of the exposure distribution closer to median values. As cities and everyday life differ a lot in specific urban contexts, places for physical activity may vary in shape, perception and use by local people. A group of researchers from eight countries in Europe, South America and Asia used the same online-photovoice application to explore places in their cities regarding physical activity in public spaces. Using the same application in eight countries with diverse local participants, we collected a rich basis for a reflection on methodological issues, the usability of the online-photovoice application and determinants of physical activity in public spaces in contrasting cities. The paper aims to provide traceable documentation of a collaborative learning activity with an online-photovoice application. In doing so, results are presented from researchers’ self-reflection as a global community of practice on how the online-photovoice approach can be applied to place analysis for healthy urban development at different places with practitioners, communities, and scientists from diverse backgrounds. The paper contributes to a broader problem understanding of physical activity in public open spaces. As one result, we find that including aspects of safety and conflict in public space is highly relevant.01A - Beitrag in wissenschaftlicher Zeitschrift
